This 90-minute webinar will focus on inTransition, a Department of Defense mandated program that supports Service members, veterans, National Guard, and reserves of all services as they move between health care systems and providers. inTransition provides resources that facilitate treatment engagement during geographic and or career transitions, to include those made to DoD, VA, and non-Tricare settings. Information about local community resources, support groups, healthy lifestyle options, benefit enrollment, and other health care resources are also available.
Service members from all branches and veterans with any category of discharge qualify to use inTransition. inTransition provides individualized, regularly scheduled, coaching and warm handoff referrals to no-cost mental healthcare in a desired setting. This free resource may be accessed by clinicians or Service members 24 hours a day, seven days a week, from anywhere in the world.
This webinar explores how inTransition assists Service members and clinicians, the research and policy behind the program, as well as policy-driven process enhancements and self-initiated improvements since program inception. It will provide clinicians and program leads within and outside the DoD the scientific rationale and policy requirements behind this important program that helps them and their patients maintain mental healthcare continuity during difficult transitional periods.
